Medicaid Service Coordination (MSC) is a service available to all Medicaid recipients diagnosed with a developmental disability as defined by the Office for People with Developmental Disabilities (OPWDD). MSC provides people with the linkage and coordination of all available services and entitlements to help them lead more fulfilling and richer lives. A highly trained and qualified service coordinator works with the person and their family to first develop an Individualized Service Plan (ISP). Also known as the Life Plan, this helps to identify life goals, wishes and desires. It is then the responsibility of the service coordinator to put this plan into action!
