The goal of the Article 16 Clinic of Lifespire is to make our services as accessible and consumer focused as possible. Our clinic provides comprehensive clinical services to individuals with developmental disabilities, which serve to bolster the consumers’ participation in the community.

Services offered include: 

1. Individual and Group Psychotherapy
2. Occupational Therapy
3. Physical Therapy
4. Speech Therapy
5. Psychiatry
6. Sexual Counseling
7. Psychometric Testing (Psychological Evaluations)
8. Psychosocial Evaluations

In order to receive these services, a referral package can be faxed to (718) 454-3583 or sent to:

The referral package must include:

• Completed Clinic Referral Form.

• Letter of consent from the CEO or Designee (for residential services).

• Letter from the Medicaid Service Coordinator outlining the current clinic services and the providers.

• Individualized Service Plan (ISP) or Comprehensive Functional Assessment (CFA) outlining the identified clinical needs of the consumer.

• Signed Consent and Payment Authorization Form.

• Legible Photocopy of Medicaid/Medicare card; updated Annual Physical Examination Report.

• Annual PPD Report or Statement from a Medical Doctor (Negative chest x-ray, free of active Tuberculosis).

• Current Medication Documents and other Relevant Medical/Medication information.

• Most recent Psychological Evaluation Report (within three years of referral)

• Most recent Psychosocial Evaluation Report (within three years of referral) and most recent Psychiatric, Speech, Occupational and Physical Therapy Reports (if applicable).
